2 months	Pentacel ( DtaP - Hib - IPV ) (Diphtheria-tetanus-pertussis-Hemophilus B-polio) PCV13 (pneumococcal vaccine) Rotavirus oral vaccine
4 months	Pentacel ( DtaP - Hib - IPV ) PCV13 Hepatitis B Rotavirus
6 months	Pentacel ( DtaP - Hib - IPV ) PCV13 Rotavirus Seasonal and H1N1 influenza vaccines - Injection Mist (second doses needed after a month if never given before)
9 months	Hepatitis B
12 months	Hepatitis A Chickenpox
15 months	Hib PCV13
18 months	DtaP MMR (measles-mumps-rubella) Hepatitis A
4 years	Kinrix ( DtaP - IPV ) MMR Chickenpox
11-12 years	Tdap (Tetanus-diphtheria-pertussis) booster MCV4 (meningitis vaccine) HPV4-Gardasil (human papillomavirus vaccine for girls only at this time)
